ACKNOWLEDGEMENT AND WAIVER

By entering an application to be considered for the KERRAZY OFFICE MAKEOVER, offered by KERR OFFICE GROUP (“KOG”), the undersigned, a duly authorized owner or officer of the business where the makeover (“the business”) is to occur hereby consents to and acknowledges the following:

  1. KOG will enter the office or business location with a mega phone and announce that they are the winner loudly.
  2. The winner (an employee or officer of the business) will be taken away in a Limo for approximately three hours with two guests of their choice from the business to join them for breakfast while the makeover is going on.
  3. KOG’s installation crew will come into the business and completely remove the office furniture, file cabinets, chairs, etc in the office receiving the makeover. The company will be responsible for a place to store their existing furniture onsite. Any off site removal of the furniture will incur a labor and mileage charge.
  4. KOG will install the new furniture. The business is responsible for having someone available at the time of our arrival at 9:00 a.m. to remove any necessary computers, cables, telephones, fax machines, office equipment, etc. It is the business’s responsibility once the furniture is in place to return all files, telephones, computers, etc, to the office.
  5. KOG will not be responsible for any damage to existing furniture once moved out of the office.
  6. The furniture that is awarded becomes the property of the business, not the individual employee or officer who submitted the pictures or the end user of the product.
  7. Pictures will be taken of the event and the business and used for advertising by KOG including in, but not limited to KOG’swebsite, newspaper, flyers, etc.
  8. KOG will conduct a ribbon cutting of the new office after the winner returns.
  9. The KERRAZY OFFICE MAKEOVER will not be a quiet process, but should be fun even though it will cause some disruption to the business’s office environment for a few hours.

The business hereby consents to and authorizes KOG to conduct the above activities in the event the business or its employee or officer is deemed the winner of the KERRAZY OFFICE MAKEOVER.
